Essential Duties and Responsibilities

Gather and review medical, psychological, and social history to assess needs and capabilities toward the least restrictive environment.

Initiate and coordinate professional assessments; present data to clients and agencies.

Complete person‑centered service plans, review progress, and ensure preferences are reflected.

Serve as primary advocate and liaison with families, guardians, caretakers, and agencies.

Provide consultation, referral, and in‑service support to staff and families.

Assist with placement planning, complete paperwork, and coordinate residential treatment plans.

Provide community outreach to reduce hospital recidivism.

Participate in after‑hours on‑call, crisis intervention, and evidence‑based practices (DBT/IDDT/FPE/IPS).

Deliver care management services to veterans and their families.

Complete OBRA assessments and coordinate as directed.

Assist with grant requirements and integrated health care linkage.

Coordinate CCBHC services and perform initial intakes.

Perform other duties as assigned.

Child, Adult, and Behavioral Care Manager Functions

Linking and coordinating services with schools, probation, DHHS, SS, and other agencies.

Family psychoeducation and participation in BTC committee.

Behavioral interventions: conduct FBA, develop behavioral plans, collect data, and report progress.

Qualifications

Bachelor's degree in Human Services or related field from an accredited institution.

Preferred licensure: RN, LLBSW, LBSW, LLMSW, LMSW, TLLP, LLP, LLPC, LPC; or BCaBA for behavior‑based manager.

Qualified as QMHP, QIDP, or CMHP when required for client population.

Veteran status required when working with veterans.

For children’s roles: 40 hours of Children’s training per year, CAFAS rater, three years’ experience with SED children and families.

Substance license preferred; MCBAP development plan encouraged.

Additional Requirements

Workable on‑call availability and ability to travel between offices and homes.

Valid chauffeur’s license if transporting clients.
