About the Role The Therapy Department at The Subacute at Autumn Lake Healthcare is seeking a fully licensed Physical Therapist Assistant to join our skilled nursing facility. Responsibilities
Implement care and interventions to improve patients’ mobility.
Provide exercise instruction, massage, gait and balance training, and other therapeutic interventions.
Demonstrate empathy, provide educational support, and document treatment outcomes.
Collaborate with the interdisciplinary team to plan and evaluate patient care.
Participate in mentorship and professional development activities.
Qualifications
Active state Physical Therapist Assistant license.
Minimum of 1 year post‑licensure experience preferred.
Strong communication and documentation skills.
Commitment to patient‑centered care and continuous learning.
