The Therapy Department at Autumn Lake Healthcare at Greenfield is looking for a full‑time PTA to fill a position in our skilled nursing facility. You'll be part of a cohesive team, providing outstanding, individualized care.
The PTA will implement care and interventions to improve patients' mobility through exercise, massage, gait and balance training, and other therapeutic interventions. By providing exercise instruction, demonstrating empathy, and documenting treatment outcomes, the PTA will enhance patients' physical abilities and well‑being.
Qualifications
An active state PTA license is required.
Unlimited employee referral bonus of up to $1,000 each.
Multiple locations offered; optional PRN coverage and potential full‑time opportunities at additional sites.
This is a direct hire position; we are not an agency.

Am I guaranteed 40 hours?
While we cannot guarantee hours, we typically offer a full caseload. Most of our facilities are close together, allowing additional hours at nearby sites.
Do you give raises?
Staff are eligible for performance reviews, where there is always an opportunity for an increase.
Why is this position open?
Therapists that leave our company do so to pursue other specialties. Many remain PRN, and we rarely see therapists moving to competing SNFs.
How does corporate support therapists on the floors?
Our Regional Directors and corporate members are therapists themselves, actively present in facilities to collaborate and support the team.
What type of mentorship or support do you provide for new grads?
We offer a comprehensive mentorship program for new grads, pairing you with an experienced therapist from day one.
Salary: $28 - $34 per hour.
