The Patient Care Manager plays a crucial role in facilitating the progression of care and closely monitors the patient's journey to guarantee that the care plan and services delivered are centered around the patient, uphold high quality, are evidence-based, tailored to the patient's needs, efficient, and cost-effective. Qualifications
What’s Required
A Bachelor’s degree in Nursing or Social Work is required. Equivalent experience may be accepted in place of a Bachelor’s degree in nursing (an RN license or social work license is mandatory). If you are a Registered Nurse, you must hold a valid license to practice in Michigan. Additionally, for individuals hired into this position after January 1, 2015, obtaining a BSN within five years of the start date is mandatory. Candidates must have recent hospital patient care experience (at least three years) and a solid understanding of hospital operations. Additional Information
Munson Healthcare requires all employees be vaccinated or have lab confirmed immunity for Measles, Mumps, Rubella and Varicella. MHC also requires all employees to receive a flu vaccine during the flu season in the year that they are hired and annually thereafter, or receive an approved medical or religious exemption.
