Work Schedule:

8 a.m. to 5:30 p.m. Monday-Thursday.

8 a.m. to 12:00 p.m. on Friday.

However, adjustments in schedule or additional hours may be required to complete duties.

JOB SUMMARY The Clinical Case Manager is part of the care team and works under the supervision of the Clinic Manager. The primary purpose of the Clinical Case Manager is to work in conjunction with all health services within the health center in order to aid in the development of a comprehensive services delivery system aimed at meeting the needs of the patients. The Clinical Case Manager is involved in continuous Quality Improvement practices and measures as set by Mainline Health Systems, Inc. and in which the practice focus is based on a Patient Centered Medical Home model of Care.

JOB DUTIES

Handle all telephone encounters for their clinic.

Be responsible for all clinic paperwork, refill requests, and notification of lab results.

Process requests from patients and staff via electronic messages.

Interact with patients and staff on a daily basis in a courteous and professional manner, addressing questions and direct calls promptly.

Communicate with management regarding any related issues.

Additional duties or responsibilities as deemed necessary.

Participate in monthly clinical staff meetings.

Serve as part of the care team; follow and implement workflows that support and sustain MHSI Patient Centered Medical Home model of care.

Willingness to attend and/or complete continuing education related to this position and/or any other topics as deemed necessary to MHSI.

Participate in daily huddles and maintain an active role within the care team.

Complete continual job training processes set forth by Mainline Health Systems, Inc.

QUALIFICATIONS

Ability to communicate clearly, take direction, and work well with a team but also be self‑motivated.

Positive attitude.

Detail oriented.

Ability to navigate and utilize electronic medical records.

One year of experience in medical field/setting.

Unencumbered Nursing License.

The ability to work in a constant state of alertness and in a safe manner.

REPORTING RELATIONSHIPS Reports directly to the Clinic Manager.

EVALUATION Annual evaluation performed by Clinic Manager with consultation of Director of Operations and/or COO.

WORK ENVIRONMENT An office environment in a controlled atmosphere building. This is an OSHA low‑risk position.

SALARY The salary for this position is negotiated between the individual and the Clinic Manager using the established salary scale. Bilingual employees receive an additional $0.50 per hour.

STATUS This is a FLSA Non-Exempt position.

This is an ADA negotiable position.
