Eliot's Community Service Agency (CSA) serves children and youth (through age 21) and their families who are in need of support in managing and coordinating services. Eliot is excited to align these essential CSA services with our Community Behavioral Health Center’s Mobile Crisis Intervention (MCI) program to offer a seamless and effective community-based continuum of care.

The ideal candidate for the

Crisis Care Manager - CSA

position is a dedicated and collaborative professional with a strong background in child and family behavioral health, crisis management, and care coordination. They possess excellent communication and organizational skills, demonstrate cultural sensitivity, and are committed to empowering families through individualized care planning, system navigation, and ongoing support.

Responsibilities:

Be embedded within the MCI program to focus on identifying and supporting families who are receiving CBHC MCI or intake services.

Serve as key contact for the youth and family to the Eliot CSA and its services.

Initiate care planning activities with all members of the family and/or caregiving system.

Conduct a comprehensive home-based assessment in conjunction with other CSA Team members inclusive of the CANS, CRAFTT (for youth 12 and older), SNCD and other tools as determined necessary, which occurs in the youth’s home or another location of the family’s choice.

Initiate planning that includes intake meetings, orient the family and the member to the wraparound process, identify strengths and needs of the child.

Develop and implement a youth and family centered Individualized Care Plan (ICP) in collaboration with the family and collaterals.

Develop an initial safety/crisis plan.

Qualifications:

Bachelor’s degree in human services or related field

Must have a valid driver’s license and reliable transportation

Experience navigating child/family service systems

Experience working with child mental health issues and crisis management

Schedule:

Monday-Friday, 10am-6pm (Flexibility is required to work earlier or later as needed to accommodate client needs.)

Eligible for 6% bilingual pay increase if fluent in needed language capacity.

$60,000 - $60,000 USD
