Job Summary DaVita Integrated Kidney Care (IKC) seeks a Full‑Time Nurse Practitioner (NP) to provide holistic, integrated care for complex kidney patients, coordinating care autonomously and collaboratively.
Position Details
Complete comprehensive health assessments
Manage complex patients focused on reducing hospitalizations and improving health outcomes
Deliver care Monday‑Friday with flexibility to accommodate patient and partner availability; no nights, weekends, or on‑call
Full‑time self‑managed schedule with no nights, weekends, or on‑call

Responsibilities
Support complex patients by navigating a challenging healthcare system
Coordinate and facilitate quality, consistent, cost‑effective care with all members of the healthcare team
Perform comprehensive health assessments and manage chronic disease care
Work autonomously while collaborating with nephrology partners and dialysis clinics
Requirements
Passion for delivering care to patients with complex, chronic illnesses
Minimum of two (2) years’ experience as a Nurse Practitioner (NP)
Current unrestricted NP license to practice in the state
Current DEA license required or eligible to obtain within 90 days of hire
Current CPR or BLS certification
Intermediate computer skills and proficiency in MS Word, Excel & Outlook
Home office, high‑speed internet and reliable transportation required
Preferred Qualifications
Experience working in Value Based Care or Population Health, including documenting and coding from a Medicare Advantage risk perspective and conducting Health Risk Assessments on high‑risk/chronically ill patients
5+ years’ experience as a Family Nurse Practitioner (FNP), Primary Care NP, or experience in Internal Medicine, Cardiology, Nephrology or Case Management
Spanish‑speaking preferred in some markets
Experience managing chronic diseases (CHF, diabetes, hypertension)
Experience managing population health outcomes such as hospitalization
Salary Range $130,000 – $155,000
