Position Details
Location: Hybrid‑Remote. Work from home and travel within an assigned geography including Dialysis Clinics and Nephrology Practices.
Complete comprehensive health assessments.
Manage complex patients, focusing on reducing hospitalizations and improving health outcomes.
Monday‑Friday schedule with flexibility to accommodate patient and nephrology partner availability.
Full‑Time self‑managed schedule.
No nights, weekends, on‑call.

Responsibilities As an IKC Nurse Practitioner (NP) you will support some of our most complex patients, assisting them in navigating a challenging healthcare system. You’ll focus on delivering holistic, integrated care to improve the quality of life for our kidney patients. You’ll work autonomously and in collaboration with all members of the healthcare team to coordinate and facilitate quality, consistent, cost‑effective care.
Requirements
Passion for delivering care to patients with complex, chronic illnesses.
Minimum of two (2) years’ experience as a Nurse Practitioner (NP).
Current unrestricted NP license to practice in the state.
Current DEA license required or eligible to obtain within 90 days of hire.
Current CPR or BLS certification.
Intermediate computer skills and proficiency in MS Word, Excel, Outlook required.
Home office, high‑speed internet, and reliable transportation required.
Preferred Qualifications
Experience working in value‑based care or population health, including documenting and coding for Medicare Advantage risk perspective and conducting health risk assessments on high‑risk/chronically ill patients.
5+ years’ experience as a Family Nurse Practitioner, Primary Care NP, or experience in internal medicine, cardiology, nephrology or case management.
Spanish‑speaking preferred in some markets.
Experience managing chronic diseases (CHF, diabetes, hypertension).
Experience with population health (hospitalization).
