Job Description Under supervision of the Director of Social Service, the
Case Manager
is responsible for providing case management, housing support to clients and monthly reporting requirements that will ensure compliance with the parameters of the program. The requirements llocated below represent the knowledge, skills, and/or ability required.
Salary: $40,000 - $45,000 annually based on relevant experience.
Key Essential Functions Client Advocacy
Conduct needs assessment and case management for assigned families
Conduct intakes, home visits and apartment inspections.
Holds bi‑weekly interviews with clients.
Maintains up‑to‑date files including client history and profile.
Provides information and referrals, arranges for provision of services, advocates and intervenes with government agencies, short‑term counseling and assists in developing service plan with goal of restoring family independence.
Coordinates workshops, seminars and other activities for general client population.
Performs other related duties as assigned.
Makes recommendations to improve level of services.
Relocation Services
Meet with Head of Household and begin filling out Section 8, NYCHA and/or Homeless Housing Application. Complete documentation in a timely manner.
Meet with Head of Household every two weeks to determine housing status and immediate needs.
Contact landlord/broker to schedule appointment to see apartment.
Follow up with landlord/broker to assure that Section 8 package or other related documents was submitted.
Schedule Head of Household for Apartment Inspection with housing site.
Follow‑up meeting with resident for Lease signing.
Follow‑up meeting, after lease has been signed, then schedule resident for moving out.
Contact Public Assistance agency to request rent checks.
Case Management and Record Keeping
Provides case management and on‑site crisis intervention.
Completes required service plans.
Educates participants with respect to good housekeeping and home‑safety practices, budgeting and landlord/tenant relations, as necessary.
Maintains appropriate documentation of all tenant contacts.
Documents monthly progress notes in the participants’ chart.
Conducts home visits to help evaluate participants’ compliance with the rules and regulations of the program and to assess the need for referral to supportive services.
Reports any alleged child neglect or abuse to the kannattaa agency or the authorities.
Education and Requirements
BA degree in relevant field
(Masters degree preferred)
Minimum of 2 years of Case Management Experience
Experience working with homeless population(s)
Housing Knowledge
Familiarity with community Resources
Excellent oral and written communication
