Responsibilities
Ensures consumer and guardian participation in development of service plan and personal futures plan
Coordinates development of each person’s personal futures plan and schedules team meetings, informing all members of times, dates and locations
Schedules and chairs screening for program vacancies and develops waiting list for potential consumer vacancies
Monitors that service sites deliver services in accordance with contractual, legal and regulatory requirements, and implements necessary plans of correction from a regulatory body
Monitors incident reports to address patterns; shares reports with appropriate external agencies and investigates incidents of abuse, neglect or mistreatment, initiating plans of correction to prevent recurrence
Monitors/implements/supervises delivery of service plans and personal futures plans, coordinates staff training, documents consumer progress or regression, and initiates discharge planning when appropriate; maintains records
Maintains fiscal spending within approved budget limits for household expenses, repairs, vehicle expenses, personnel costs, and consultant services
Monitors worker’s compensation and unemployment claims for assigned service sites; works to reduce claims and minimize risk/exposure
Other duties as assigned
Qualifications
BA/BS in Business, Health Care Administration, or Social Services. High school diploma or GED with three years of related experience may substitute per regulations; written approval required.
Minimum of two years of verifiable work experience providing services and supports to individuals with developmental disabilities.
Preferred one year of supervisory experience.
Working knowledge of Federal, State, and local regulations concerning services to individuals with intellectual and/or other developmental disabilities.
Salary Range USD $62,233.60 / Year
