Job Summary The Case Manager III performs complex senior‑level case management for patients admitted to the state hospital. The role involves developing individualized recovery plans, coordinating with physicians and other providers, documenting progress, and ensuring compliance with clinical and regulatory standards.
Responsibilities
Oversee case management activities for all patients admitted to the state hospital.
Develop and monitor patients’ individualized recovery plans, including therapeutic goals and discharge or competency restoration objectives.
Conduct 1:1 meetings and serve as a recovery team liaison, ensuring clear communication with patients and their families.
Work with physicians to align treatment plans with medical orders.
Document case progression in the electronic medical record and maintain timeliness of required documentation.
Participate in staff and patient reviews, recommending therapeutic activities and observing treatment outcomes.
Present case information at civil and forensic court hearings as needed.
Coordinate transition planning and continuity of care for intra‑agency transfers, external providers, and the Local Mental Health Authority.
Assist with program evaluation, policy development, and disaster response or COOP activation if required.
Maintain compliance with Joint Commission (TJC) and CMS standards, completing required documentation within 72 hours.
Qualifications
bachelor’s degree and at least six months of case‑management or social‑services experience, OR
associate’s degree and at least 24 months experience, OR
no degree and at least 48 months experience; plus a minimum of 3 years overall experience in case management or social services.
knowledge of community resources, case‑management principles, and program policies.
ability to communicate effectively orally and in writing.
ability to collect and analyze program data for quality monitoring.
proficiency in coordinating client services and supervising the work of others.
valid Texas driver’s license; out‑of‑state license holders must provide a certified driving record.
Additional Information
Must pass a fingerprint criminal background check, pre‑employment drug screen, and various registry checks.
Applicants must be at least 18 years of age; Waco Center for Youth applicants must be 21.
Compliance with HHSC immunization policy and state hospital procedures is required.
Flexibility for overtime and extended hours may be required.
