About the Position The Olmstead Specialist will serve a specialized role and lead efforts specific to the transition process for residents at JIRDC who are under a MOA and/or on the Community Transition List (CTL). The role serves as the primary liaison between the resident’s treatment team, legal guardian, and care coordinator/MCO in matters regarding community re‑entry.
Responsibilities
Facilitate progress of residents’ transition by scheduling and leading transition meetings that involve the treatment team, guardian, MCO, and other relevant participants (e.g., Vocational Rehab, identified provider).
Maintain up‑to‑date knowledge of available community options and prerequisites needed to access them.
Keep JIRDC social workers and admissions committee members abreast of service trends and community options pertinent to the population served.
Provide counseling to legal guardians and residents regarding community options that best align with the resident’s needs.
Review facility referrals to ensure adequate exploration of community‑based supports/resources before initiating the screening process.
Build trusting relationships with stakeholders and effectively address and resolve issues that interfere with the transition process.
Report directly to the Director of Professional Services.
Qualifications & Experience
Master’s degree in social work from an accredited institution; or Bachelor’s degree in social work with one year of directly related experience; or Master’s degree in a human services field with one year of directly related experience; or Bachelor’s degree in a human services field with two years of directly related experience; or an equivalent combination of education and experience.
Documented knowledge of MCOs, general community supports for people with developmental disabilities, and transitions of institutionalized residents with IDD back into the community.
Considerable knowledge of community IDD resources, service systems, and waiver programs.
Demonstrated experience with Olmstead efforts and/or knowledge of the integration mandates of the North Carolina Persons with Disabilities Protection Act and the Americans with Disabilities Act.
Salary and Benefits Salary Range: $43,738.00 – $76,542.00 Sign‑on Bonus: $4,000 (subject to 2025‑2026 budget approval)
Sign‑On Bonus Eligibility A newly hired permanent employee must work 30‑40 hours per week to qualify. Employees eligible for less than 40 hours per week receive a prorated rate. The following are
not
eligible for the sign‑on bonus:
Had an employee position in North Carolina State government, the judicial system, or the University of North Carolina system within the last 12 months.
Previously received a sign‑on bonus from any state agency.
DHHS paid any contract placement or recruitment fees.
