Therapy Discharge Practitioner – based in the Discharge team at Calderdale and Huddersfield NHS Foundation Trust (CHFT). The closing date is 14 January 2026 This post will work over a 7 day service and may require weekend and bank holiday work on a rota basis. There will be 2 permanent positions and 1 Secondment for 12 months. Main duties of the job

The post holder will maintain momentum of discharge planning throughout the entire process, supporting and working in partnership with the MDT. The post holder will act as a Trusted Assessor for health and social care pathways and will complete relevant referral pathway documentation, such as Trusted Assessor forms. Develop education and understanding about factors that influence health and ill-health and knowledge of the wider health and social care network of services available to support patients (e.g., anatomy and physiology). This may include specialist clinical tasks such as assessing for walking aids or equipment to support transfers, ordering equipment based on assessments, and developing these skills through competency training. The post holder may undertake lone working (sometimes within a patient’s own home) in line with Trust Policy. About us

We employ more than 6,500 staff delivering compassionate care from Calderdale Royal Hospital and Huddersfield Royal Infirmary, as well as in community sites, health centres and patients’ homes. We also have almost 150 volunteers. We provide a range of services including urgent and emergency care; medical and surgical specialties; maternity; gynaecology; critical care; paediatrics; end of life; and outpatient and diagnostic imaging services. We also provide community health services including sexual health services in Calderdale and local health centres (e.g., Todmorden Health Centre and Broad Street Plaza). They are part of the NHS, with decisions driven by independent boards and governed by their members and local communities. Job responsibilities

Formal knowledge and experience

– Formal qualifications and experience expected of a new candidate (see person specification). Supporting patients and service users

– Supporting patients and their families and providing care throughout their care pathways. Includes assessing for, ordering, and issuing basic equipment and discharge planning in conjunction with the MDT and registered practitioners. Lead onward referrals to other health-related teams within CHFT/external agencies. Undertake lone working in line with Trust Policy; complete administrative duties and general patient care across departments as required; participate in manual handling training and contribute to moving and handling of patients; undertake clinical, technical and scientific roles as appropriate. The underpinning knowledge and practice, support and interventions required of support staff to safely assist service users in meeting their optimum potential. This includes determining a treatment and/or discharge plan based on a needs-based assessment, recording and sharing information with the MDT, and working with wider Health and Social Care teams.

Note:

tasks may vary by area and will depend on the specific service requirements. Carry out baseline assessment related to health and social care support; request simple services or alterations to care packages; liaise with Gateway to Care and Single Point of Contact and social services where appropriate. Communication and information management; work with the MDT to complete discharge documentation and onward referrals; demonstrate negotiation and conflict resolution as needed; support patients and relatives during stressful times; ensure accurate information on electronic systems and obtain consent for other services; ensure compassionate care and safe, inclusive environments. Adhere to equality, diversity, safeguarding, personal wellbeing and safety, duty of care and candour; ensure documentation is accurate, up-to-date and shared appropriately; comply with Trust policies and review relevant systems for safe discharge and care. Participation in research, audit, evaluation and service improvement projects; attend mandatory training and maintain updates to additional training relevant to the post (e.g., medical devices, competency training). Qualifications/training

5 GCSEs including Maths and English (grades 9-4 or A*, A, B, C) or equivalent level 2 qualification; equivalent working experience will be considered. Level 5 Apprenticeship or will complete when in post. Band 4 Calderdale Framework Competencies completed or to be completed when in post. Willingness to participate in competency-based training around rehabilitation and health and social care assessment. A-Levels or Health and Science A-Level or equivalent level 3 qualifications; Foundation degree in Health and Social Care; Care Certificate; previous competency-based training in a relevant field. Knowledge, Experience, Expertise

Understanding of illness and disability needs and issues; recent experience in health and social care; multidisciplinary team experience; knowledge of recovery concepts and long-term conditions’ impact on health and wellbeing. Basic computer skills; experience in hospital, patient homes and community settings; experience teaching and basic counselling; experience completing needs-based assessments and care plans; experience with service users with life-limiting conditions and challenging times; ability to work independently and make decisions; knowledge of equipment used to support discharge; understanding of wider inter-agency issues and relevant legislation and policy; knowledge of local community resources to support living well at home.
