Case Manager (CT)

in Salt Lake City, Utah!!

The Case Manager - CT serves clients who are homeless or at risk for homelessness by providing a range of services to reduce the chances of veterans becoming homeless once they leave Valor House. Veterans involved in the Clinical Treatment and Special Needs programs are at increased risk, and are struggling with substance use and/or mental health diagnoses. This position involves both direct and indirect services including goal setting, needs assessments, tracking treatment progress, and connecting clients with a range of resources (e.g., housing, education, employment and medical). The Case Manager - CT assists in other administrative tasks and works in concert with the priorities of the Executive Director, the Senior Management Team, and the Board of Directors. The overall goals of this position are to (a) provide high-quality care for veterans, (b) coordinate services for case management grants and programs, and (c) provide support for the clinical and administrative staff.

Pay Rate $46,353.60 per year Monday - Friday flexible hours

Education and Experience A minimum of a bachelor's degree in social work or related experience is required. An active license as a Social Service Worker in the State of Utah is preferred. Veteran or active Reservist or National Guard member preferred. Case management experience is preferred. Experience in the substance-abuse field is preferred.

Duties include but not limited to: Performs needs assessments with clients and delivers person-centered interventions based on both Veteran need and capacity. Collaborates with Valor House clinical staff in determining additional needs for treatment as well as barriers which may need to be addressed both during and after treatment Performs regular review of medication management adherence for applicable clients and works collaboratively with the larger treatment team to address concerns Develops case management plans with each client and meets regularly to review progress Collaborates with outside agencies to connect clients with employment services, education resources, healthcare programs, housing programs, and other relevant community resources Uses the agency electronic health record to enter case-management notes, client information updates, and other relevant client information Utilization of HIMIS, excel tracking system, and other relevant data management systems Problem-solving and critical thinking Grant specific knowledge/ specific population knowledge (veterans)
