Registered Nurse (RN) - Triage

Mountain Community Health Partnership (MCHP) is seeking a Registered Nurse (RN) to fill the Triage position. The Triage Nurse enhances patient care by serving as a key communication link between patients and providers. Triage manages incoming patient calls, provides evidence-based medical advice within scope, coordinates care, and supports patients in management acute and chronic conditions. Key Responsibilities:

Respond to patient calls promptly and professionally, routing calls as necessary. Provide medical advice following NCBON standards and MCHP protocols. Schedule routine and urgent appointments based on clinical need. Document all patient interactions accurately in the EHR. Process medication refills. Assist in clinical areas when needed. Coordinate care by tracking test results, consult notes, and communication with external partners. Maintain struct patient confidentiality in accordance with HIPAA. Qualifications:

Education: Graduate of an accredited nursing school. Active RN License in North Carolina. Current BLS Certification. Experience: Minimum of one year in medical/surgical or clinical setting preferred. Strong communication, interpersonal, and organizational skills. Ability to be flexible, adaptable, and comfortable working with diverse populations. Bilingual, preferred. Benefits:
