Registered Nurse - Resident Assessment Coordinator / Minimum Data Set Coordinator Pay Base pay range: $72,392.00/yr - $144,373.00/yr

Overview The Cincinnati VA Medical Center is seeking a Resident Assessment Instrument (RAI) / Minimum Data Set (MDS) Registered Nurse (RN) in the Community Living Center. The RN will assist in directing the provision of nursing education — orientation — competencies and providing quality improvement and outcomes utilization consultation.

Qualifications

English Language Proficiency

Graduate of a school of professional nursing approved by an accrediting agency such as ACEN or CCNE

OR individuals attending a master’s level bridge program who have completed coursework equivalent to a bachelor’s level degree in Nursing may have an opportunity to become registered as a nurse with a state licensing board prior to completion of the bridge program

Upon achievement of a State license – the individual may be appointed on a temporary basis and later converted to a permanent appointment upon successful completion and graduation from the bridge program (Reference VA Handbook 5005 – Appendix G6)

OR graduates of foreign schools of professional nursing holding a current, full, active, and unrestricted registration will meet the requirement for appointment as a nurse who has completed an approved school of professional nursing (foreign education higher than associate degree requires formal degree equivalency validation from a recognized equivalency evaluation accepted by VA such as ICD)

Current, full, active, and unrestricted registration as a graduate professional nurse in a U.S. State – Territory or Commonwealth (i.e., Puerto Rico) or the District of Columbia

Physical Requirements: A physical examination will be conducted based on the physical requirements of the position; required visual acuity, keen hearing, clear distinctive speech, and manual dexterity. Positions may require extended walking, standing, stooping, sitting, bending, pulling, and pushing, and transferring patients and objects. Exposure to infected patients and protected clothing may be required. Occasional exposure to combative patients related to delirium, dementia, or psychiatric disorders.

RAI/MDS Coordinator: Responsible for providing competent, evidence-based care to assigned residents and overseeing licensed vocational nurses/nursing assistants as appropriate.

Assist in directing the provision of nursing education, orientation, competencies, and quality improvement and outcomes utilization consultation.

Demonstrate leadership in delivering and improving holistic care through collaborative strategies.

Code the sections of the Resident Assessment Instrument (RAI)/MDS Assessment and the Care Area Assessments (CAAs) assigned to nursing, adhering to current CMS and VHA timeframes.

Participate in the CLC Interdisciplinary Team care plan meetings to review, communicate, and monitor residents’ care plans.

Collaborate with the CLC Resident Assessment Coordinator (RAC) to educate staff on RAI/MDS completion.

Support quality improvement activities based on CLC quality management data.

Participate in ongoing survey readiness activities in the CLC.

Document and take corrective action when notified by CLC management, charge nurse, or designee of any discrepancies in coding or documentation.

Provide peers with informal constructive feedback for improvement.

Foster a safe and supportive environment conducive to professional development.

Support colleagues through knowledge sharing to provide safe, quality nursing care.

Supervisory Controls: Accountable to the Associate Director of Patient Care Services (ADPCS).
