The Practice Nurse is to support healthcareprofessionals within the organisation and across the wider network to enhance integrationwith community, secondary, and social care services.
This role is responsible for the delivery of evidence-basednursing services, working as part of the practice multi-disciplinary team,delivering care within their scope of practice to the entitled patientpopulation.
Working autonomously, the PracticeNurse will be responsible for a number of clinical areas such as public healthand screening programmes, including infection prevention and control, chronicdisease management, immunisations, health promotion and prevention, wellman/woman clinics and cervical screening, Furthermore, they embed populationhealth management approaches within general practice, targeting healthinequalities and improving access.
The post-holder will be an integralpart of the general practice team. They are to participate in formaltraining events and to promote best practice in their area of expertise as wellas supporting the practice management team in the reviewing and delivery ofclinical policy and procedure.
Main duties of the job
Lead for Infection Prevention Control (IPC), working to the IPC Policy, attending meetings and providing updates to the team. Support in long-term condition management within the organisation, including being involved with quality improvement and assurance initiatives Support public health and screening programmes, including immunisations, vaccinations, and cervical screening and also provide opportunistic health promotion advice Embed population health management approaches within general practice, targeting health inequalities and improving access As required, provide routine nursing care to patients in accordance with clinical based evidence, NICE and the NSF Support with the development, and updating of practice protocols/guidelines and procedures locally Support in the delivery of enhanced services and other service requirements Be aware of duties and responsibilities regarding current legislation and adhere to practice policies and procedures on Safeguarding Adults and Safeguarding Children Contribute to and embrace the spectrum of clinical governance Attend a formal appraisal with their manager at least every 12 months. Provide wound care (ulcer/Doppler, etc.) to patients Contribute to practice targets (QOF, etc.), complying with relevant guidance Chaperone patients where necessary Support the team in dealing with clinical emergencies About us
We are a growing practice on the outskirts of Norwich, residing in the popular location of Old Catton.Our current patient list size is approx. 8,700 patients. We are led by three GP partners and have one salaried GP, as well as a varied team of amazing support staff. More recently we have become a teaching Practice so you will see students from time to time, as well as a resident GP registrar. The team at Old Catton Medical Practice is small and very friendly, with many longstanding members of staff. Once aperformance/training objective has been set, progress will be reviewed on aregular basis so that new objectives can be agreed Provide wound care(ulcer/Doppler, etc.) to patients Maintainaccurate clinical records in conjunction with extant legislation ensuring thatSNOMED CT codes are used effectively Contributeto practice targets (QOF, etc.), complying with relevant guidance Maintainchronic disease registers and implement an effective call/recall system Maintain a clean, tidy,effective working area at all times Chaperonepatients where necessary Supportthe team in dealing with clinical emergencies Liaisewith external services/agencies to ensure the patient is supportedappropriately (vulnerable patients, etc.) There may be, on occasion, a requirement tocarry out other tasks, this will be dependent upon factors such as workload andstaffing levels In addition to the primary responsibilities,the Practice Nurse has the following wider responsibilities: Develop, review and implement nursing protocolsin conjunction with the partners Participate in local initiatives to enhanceservice delivery and patient care Support and participate in shared learning inorder to improve patient care Support with the planning and implementation ofchanges within the nursing team, enhancing the provision of services topatients Person Specification
Qualifications
Current NMC registration with active PIN Additional qualifications in the monitoring and management of any of the following area's would be advantageous: Asthma COPD Immunisations Mental Health Experience
Working as part of a multi-skilled nursing team. Experience in the following area's would be advantageous: Working in a primary care setting. Using SystemOne and Ardens. Using and reporting on INR Star Women's health, to include contraception advice and administration as well as cervical screening Disclosure and Barring Service Check
