Mental Health Enhanced Liaison Clinician

Wereexpanding our Mental Health Liaison Service across Worcestershire RoyalHospital and Alexandra Hospital, Redditch, and were looking for dynamic,compassionate Band 6 Mental Health Practitioners to join our team, fulling apart-time 31.5 hour post.
About the Role Provide 24-hour mental health support to A&E departments
Deliver care to individuals of all ages with suspected or diagnosed mental health conditions
Support prisoners transferred from His Majestys Prisons with specialist mental health assessments
Work within a multi-disciplinary team offering a full spectrum of mental health specialties

This is a part time role working 31.5 hours per week.
Main duties of the job The team will provide comprehensive assessment, advice, ongoing support & monitoring of patients with mental health needs across all wards. We provide psychological support to patients and carers referred to our services in conjunction with ongoing physical health needső ensuring holistic care throughout the patients stay in hospital. Band 6 clinicians work as enthusiastic team members across oge Acute hospitals, assessing patients in an autonomous, systematic way, developing and reviewing treatment plans, advising and supporting Acute staff and referring patients on through established referral pathways. Clinicians also contribute to the development of training packages & to deliver bespoke training to Acute Trust staff. We provide guidance, support and education to frontline NHS staff. This is an opportunity to be part of a developing team which will be researched based in design according to the need of the Acute inpatientPreparing population

Job responsibilities Maintain high standards of patients care, ensuring patients individuality and dignity is maintained at all times, while promoting independence. Assessing and supporting individuals of any age including children and young people, working age and older adults presenting with mental health concerns. Completing, accurately documenting and appropriately sharing complex psychosocial assessments, including mental state examination, risk assessment and crisis interventions. Liaising with carers, families.Entities and professionals.
Able to actively and creatively engage patients and carers, providing psychological interventions and signposting onward referrals for individuals ensuring holistic care in conjunction with ongoing physical health needs.
Work closely with care coordinators, social workers and all primary health care team members and all others concerned to facilitate safe discharge or transfer of patient care.
Act as a role model to colleagues,ísimo always seeking to maintain the highest standards ofitti professionalism. Use initiative and take responsibility for themselves and the quality of your work and the service you provide to patients, carers and families.
In partnership with patients, carers and other agencies (both NHS and voluntary) create effective, dynamic and robust risk management and crisis plans tailored to individual need, following full psycho-social assessments.
Offer education and information to patients and carers as appropriate in regard to mental health issues and illness, encouraging empowerment, independence and informed decision making.

Able to meet the travel requirements of the role
Experience
At least 2 years post qualification experience in ännu appropriate mental health setting
Working within and utilising a supervisory framework
Undertaking regular multi-disciplinary assessments
Undertaking mental state and self-harm assessments
Assessing and managing risk
Working within the Care Coordination framework
Supervision of junior staff and students
Experience of assessing people who deliberately self-harm and those diagnosed with personality disorder.
Experience of working in a liaison or crisis and home treatment service.
Skills & Abilities
Ability to challenge poor behaviour and provide positive role modelling for good behaviour
Able to work flex ادام within the framework of liaison services. Able to organise self and workload.
Willingness to train in a variety of areas and new initiatives.
Advanced assessment skills
Teaching skills
Knowledge
Good working knowledge of mental Health Act legislation and Mental Capacity Act 2005.
Clinical Governance, Risk Assessment and Adult/Child Protection Legislation and NSFs.
National and local health and social care strategies.
Qualifications
Registered Mental Nurse, Occupational Therapist or Social Worker
Evidence of on-going continuing professional development
