Social Worker - HCBC Child & Adolescent Team

Carver County is seeking a results-driven professional committed to fostering collaboration, growth, and positive outcomes in a Social Worker role. This person will be on a team of long-term care child and adolescent case managers primarily served on MN Waivers. Under the direction of the Home and Community Based Care Supervisor, this position is assigned to the Child & Adolescent long-term care team. The Social Worker plays a key role in supporting children and adolescents with complex needs by coordinating individualized services that promote stability, independence, and well‑being. Strong collaborative partnerships with team and other departments is necessary. This position will work onsite and in the field during the probationary period, with possibility of hybrid/remote work at a later date.

Responsibilities
Development of service plans in collaboration with the interdisciplinary team
Educating individuals/legal guardians of service options and eligibility requirements
Assisting in the identifying and connecting the child/family with appropriate providers, resources, and supports
Facilitate access to services through coordination of referrals and authorizations
Monitor and evaluate effectiveness of the services through routine contact, documentation, and plan updates
Complete annual interdisciplinary reviews to ensure alignment with identified needs and goals
Maintain accurate and timely documentation in accordance with agency policies and regulatory requirements
Collaborate with internal and external partners to ensure continuity of care for the needs of the individuals served
Maintain current and changing training requirements to meet the demands of the job
Qualifications
BA or BS in Social Work, Psychology, Sociology or related area.
Minimum of 2 years previous experience in human/social services.
Requires a valid MN driver's license or evidence of equivalent mobility.
Must successfully complete and maintain eligibility to provide direct care to clients in accordance with Minnesota Statutes §245C.03.

The highest ranked candidates meeting minimum qualifications will be recommended for the scored oral examinations (interviews). Final selection for the position will include successful completion of written exercise, oral interview(s), reference checks, and background investigation, including 604 verification and MNDHS NETStudy 2.0 background.
