Case Management Team !

As an Active Case Manager, you’ll play a vital role in delivering compassionate, person‑centered service and support coordination to individuals receiving Medicaid Waiver and State General Fund services. Your days will be filled with:

Conducting in‑home visits

to complete assessments, monitor service provision, and facilitate service plan meetings.

Engaging with families and providers

in the community to build trust and foster strong partnerships.

Collaborating at our Aurora office , sharing insights and strategies with your team and other departments to enhance service delivery.

Working from home , which includes hosting virtual Zoom meetings with individuals and their teams, documenting and coordinating services, and participating in virtual office hours, team meetings, and more.

Driving between locations , spending time in the community and at your home or chosen remote workspace. This dynamic, hybrid role offers a fast‑paced and rewarding experience where you help weave together services that empower the lives of individuals with disabilities.

Schedule Enjoy a 4‑day, 40‑hour workweek (Monday–Thursday or Tuesday–Friday) with the flexibility to work remotely, in‑office, and in the field.

This position requires frequent in‑person presence to support essential responsibilities such as monitoring assessments and other duties in alignment with state regulations and company policies. Candidates should be prepared to travel within our catchment area (Arapahoe, Douglas, and Elbert counties) on a weekly basis to fulfill these requirements. Flexibility and reliability in meeting these obligations are critical to success in this role.

Qualifications

Five (5) years of experience in the field of Long‑Term Services and Supports (LTSS), which includes Developmental Disabilities; or some combination of education and relevant experience appropriate to the requirements of the position.

Valid Colorado motor vehicle license and proof of motor vehicle insurance.

Driving requirements – no suspensions and no more than two (2) tickets in the past three (3) years.

Upon request, qualifying need, and approval, driving requirements may be waived in lieu of reliable transportation.

Preferred

One (1) year of experience in developmental disabilities / long‑term services and supports.

Additional Responsibilities

Schedule, facilitate, document, and complete follow‑up for all required meetings; disseminate updates and obstacles to the necessary team members.

Make collateral contacts with community resources such as law enforcement and medical personnel to obtain additional information, coordinate services, or secure assistance.

Ensure the accuracy and timely entry and distribution of required documentation to all necessary team members, including, but not limited to, providers, individuals, and family members.

Provide case management services and support to temporarily assigned individuals; support with department and team coverage as needed.

Complete required documentation related to monitoring activities and other targeted case management activities; evaluate the need and effectiveness of services requested and delivered.

Coordinate support in emergency and crisis situations.

Complete assessments to determine eligibility for services/supports and identify individual strengths, needs, concerns, and preferences through interviewing, observing, and utilization of standardized tools.

Maintain knowledge of state/federal regulations, policies, and procedures regarding long‑term services and supports, Medicaid waiver and state‑funded programs and other community resources.
