Monday???Friday, 8:00 AM ??? 5:00 PM EST Pay Range:
$35-$42/hr, plus mileage reimbursement Work Environment:
Remote role with significant fieldwork Travel Requirements:
Approximately 80% travel; fieldwork radius of 2.5???2.75 hours one way from home residence for face-to-face member visits Mileage and lodging (if needed) reimbursed Quarterly in-person team meetings/training sessions required Home Health Field Care Manager Responsibilities Manage a caseload of healthcare members with
LTSS (Long Term Services and Supports)
needs Conduct
monthly and quarterly member contact , primarily through in-person visits and telephonic outreach Perform comprehensive
member assessments
and documentation Complete assessments with members, caregivers, and providers to evaluate client status, support systems, and service needs for care plan development Monitor delivery of services and follow up with members, caregivers, and providers to ensure continuity of care Authorize and coordinate referrals for services Ensure provider services are delivered without gaps and identify functional deficiencies in plans of care Coordinate informal and voluntary services to integrate into member care plans Collaborate with discharge planners, physicians, and other stakeholders to ensure appropriate discharge planning and coordination of acute and long-term care services Assist members with filing and resolving complaints and appeals Home Health Field Care Manager Qualifications Required: Bachelor???s degree RN and/or LCSW / LCSW-A / BSN Valid driver???s license
and reliable transportation Required: 2???4+ years of physical health care management experience Preferred: 4???6+ years of physical health care management experience Preferred backgrounds include hospital case management, home health, discharge planning, or long-term care Home health experience strongly preferred Proficiency with Microsoft Office Experience using electronic medical/health record systems Bedside care management experience Disqualifiers No psychiatric counseling???only backgrounds (must have physical bedside case management experience) No candidates directly out of nursing school No nursing home???only or rehab???only backgrounds If you are interested in this
