LTSS Service Care Manager Location
Lehigh Acres, FL Assignment Details
Duration: 3 months with intent to convert Schedule: 8:00 AM - 5:00 PM, Monday-Friday Work Arrangement: Field-based with remote work; extensive local travel required Position Summary
Assists in developing, assessing, and coordinating holistic care management activities to enable quality, cost-effective healthcare outcomes. May develop or assist with developing personalized service care plans/service plans for long-term care members and educates members and their families/caregivers on services and benefits available to meet member needs. Background & Context
This role supports long-term care members within a state health plan, focusing on field-based assessments, care coordination, and ongoing member engagement. Key Responsibilities
Evaluates the needs of the member, the resources available, and recommends and/or facilitates the plan for the best outcome Assists with developing ongoing long-term care plans/service plans and identifies providers, specialists, and community resources Coordinates between members, families/caregivers, and care providers to ensure services are accessible Provides resource support related to employment, housing, independent living, justice, and foster care Monitors care plans, member status, and outcomes and recommends updates based on needs Interacts with long-term care healthcare providers and partners to ensure member needs are met Collects, documents, and maintains member information to ensure regulatory compliance Performs home and site visits to assess member needs Educates members and families on procedures, services, referrals, and healthcare benefits Provides feedback to leadership on opportunities to improve quality and cost-effective care Performs other duties as assigned Complies with all policies and standards Education & Certification
Required: Bachelor's degree and 2-4 years of related experience (bachelor’s degree within healthcare realm such as Psychology, Sociology, etc.). Preferred: None. Licensure
Required: Valid driver's license. Preferred: None. Required Experience & Qualifications
2+ years of Care Management experience (field experience is a must). Caseloads of 50, 60, 70 members; bonus for geriatric caseloads. Long-Term Care Medicaid experience. Medicaid / Medicare experience. Ability to manage high case load. Adaptability to fast-paced environment and new processes. Comfort with IT support, field equipment, office space, and timely documentation (within 24 hours). Experience with electronic medical records. Home Health Experience. Strong organizational and time-management skills (calendar management, documentation). Nice to Have
Discharge Planning. Experience with TruCare software. Bilingual skills are preferred; note if required via the notes section. Disqualifiers
Not having field experience. Not having previous experience with high caseloads. Key Hard Skills
Field case management (2 years). Technology savvy. Effective time and documentation management. Position is offered by a no fee agency.
