Position: Physical Therapist – Home Health Service Area: Naples, FL Schedule: Monday–Friday, full-time (40 hours/week) with participation in rotating weekend and holiday on-call coverage A growing home health provider is seeking a full-time Physical Therapist to support patient care in the Naples area. This role offers autonomy, flexibility, and the ability to work as part of a collaborative, interdisciplinary team. Position Highlights: Flexible scheduling with reasonable caseload expectations Supportive clinical environment with a focus on quality patient outcomes Opportunity to work independently while still being part of a cohesive team Compensation & Benefits: Competitive salary based on experience Comprehensive benefits package including medical, dental, vision, life insurance, and retirement with match Generous PTO Responsibilities: Conduct comprehensive assessments, including OASIS, at appropriate intervals Develop and adjust individualized care plans based on patient needs and progress Deliver evidence-based PT interventions in the home setting Educate patients and caregivers on recovery goals, self-management strategies, and injury prevention Coordinate with nurses, therapists, and other team members to ensure integrated care Qualifications: Current Florida Physical Therapist license At least 1 year of experience as a licensed PT (home health experience preferred) Familiarity with EMR systems; Homecare Homebase experience is a plus Strong communication, clinical judgment, and time management skills Willingness to cover weekend/holiday rotations as part of a shared schedule
