Job Summary
The RN Case Manager facilitates appropriate care coordination for patients and their families when clinical issues influence effective utilization of inpatient or outpatient treatment. Responsibilities include organizing and facilitating access to tests, procedures, and discharge planning, liaising with other departments and agencies, recommending evidence‑based interventions, providing training and education about case management services, collaborating with Social Work Case Managers, and participating in departmental coverage.
Qualifications
BSN or equivalent
Registered Nurse (RN) licensure
Minimum 3 years of experience providing patient care
No supervisory responsibilities
Physical Requirements
Intermittently sitting at a computer workstation for extended periods
Occasionally standing or walking with objects weighing up to 25 lbs
Occasionally lifting, pushing, and/or pulling objects weighing up to 50 lbs
Exposure to loud noises, temperature extremes, hazardous chemicals, cuts/punctures, blood‑borne/airborne pathogens, and possibly combative or violent individuals
Shift
Monday through Friday, 8:00 am – 4:30 pm; occasional weekends or holidays required.
Skills / Knowledge / Abilities
Clinical work experience with RN licensure
Understanding of Swanson’s nursing model
Direct patient care experience
Exceptional written and verbal communication skills
Independent work ability and collaborative teamwork
Proficient in Microsoft Office and other computer systems
Above average customer service and active listening skills
Patient advocacy to promote continuity of care and quality outcomes
