Salary:

$21-23 an hour .

Position Summary The After Care Program assists and supports youth and families navigating post‑discharge obstacles. The Aftercare Specialist will work effectively with clients, biological or foster families, staff, the Department of Social Services, and community contacts, utilizing Family Centered Practice principles.

Responsibilities

Travel to and from the residences of youth and families to conduct aftercare casework contacts.

Collaborate with social services and other community providers and agencies.

Work effectively with biological and foster families.

Assess the needs of children in the program and support with appropriate services.

Serve as the aftercare worker for children discharged from the QRTP, ensuring that all six months of family‑based aftercare support includes:

Individual, group, and family mental health counseling follow‑through

Case management

Educational follow‑through

Transportation

Extra‑curricular activity and community support

Crisis management

Parent support

Housing support

Ensure that minimum standards of contact are made.

Maintain documentation of contacts, including progress on goals, outstanding needs, and strategies to address them.

Communicate professionally with families, the Department of Social Services, and the support team.

Work flexible hours to accommodate client and family needs.

Requirements Education

Bachelor’s degree in human services or related field required.

Experience

Experience with children and families preferred.

Driver’s license and ability to maintain insurability throughout employment required.
