Patient Care Navigator (PCN) Responsibilities

Serve as the primary point of contact for clients needing assistance navigating medical, behavioral health, and social service systems.

Conduct trauma‑informed outreach, intake, and needs assessments.

Support clients in scheduling medical, dental, behavioral health, and specialty appointments.

Provide appointment reminders, transportation coordination, and follow‑up after visits.

Assist clients in understanding care plans, medication instructions, and provider recommendations in partnership with interpretersדים

Track referrals and ensure timely completion of services.

Trauma-Informed Support Build trusting, supportive relationships with clients using culturally sensitive and trauma‑informed practices.

Recognize signs of distress and collaborate with clinicians to ensure safety and appropriate intervention.

Empower clients through education on rights, available services, and self‑advocacy strategies.

Social Service Linkage

Connect clients with community resources including legal services (asylum, immigration support), housing, food security programs, financial assistance, and victim services.

Collaborate with community agencies, advocacy groups, and refugee/immigrant‑servADE organizations.

Assist clients in completing applications for benefits, medical insurance, and community support programs.

Program Support & Administration Maintain accurate, timely documentation in the EHR and program databases അനകല.

Participate in multidisciplinary case conferences with clinicians, case managers, legal partners, and program leadership.

Assist with data collection, reporting, and quality improvement initiatives.

Support outreach and education efforts to community partners about torture survivor services.

Uphold confidentiality, HIPAA standards, and ethical guidelines.

Required Qualifications

Associate’s or Bachelor’s degree in Social Work, Human Services, Public Health, Psychology, or related field; or equivalent experience.

Minimum 1–2 years of experience working with vulnerable populations, including refugees, immigrants, or survivors of trauma.

Multilingual skills (e.g., Spanish, Arabic, Haitian Creole, Dari/Farsi, Somali, Swahili, French).

Knowledge of trauma-informed care, cultural humility, and social determinants of health.

Ability to work effectively with professional interpreters.

Proficiency in electronic health records and basic computer applications.

Preferred Qualifications Experience working in a community health center or non‑profit setting.

Lived experience as a refugee, immigrant, or survivor of trauma is valued.

Knowledge of asylum processes, victim assistance programs, or refugee resettlement systems.

Summary The Patient Care Navigator (PCN) supports survivors of torture, trauma, and persecution in accessing comprehensive, culturally responsive health, behavioral health, and social services. This role helps clients understand the care system, removes barriers to services, coordinates referrals, and promotes healing and stabilization.
