Join Clinical & Support Options, Inc. to apply for the Care Coordinator, ICC/FIT, Greenfield role. CSO is looking for a Care Coordinator for our Intensive Care Coordination (ICC)/Family Based Intensive Treatment (FIT) team in Greenfield.
Intensive Care Coordination is based on the well‑known principles of Wraparound Care that respects and incorporates the needs and choices of the family with a focus on family culture and strengths. The Care Plan Team is chosen by the family and the team helps inform the family on the resources, supports and interventions that can support the family's goals.
Coordination of Care has shown to have better outcomes in improving the lives of children and families and is a key component of our children's behavioral health system. Care Coordinators work as a team with a Family Partner, an individual with lived experience parenting a child with emotional/behavioral needs.
Starting Salary: $38,000-$44,000/year
Responsibilities
Coordinate multiple services/supports for children and youth under the age of 21 with emotional/behavioral needs to develop an integrated Care Plan.
Facilitate Care Planning and learn the Wraparound Model of Care.
Act as a liaison with partnering agencies, state agencies, school personnel, etc.
Meet with youth and families in their home, at the office and in the community.
Provide referrals and linkages to appropriate services.
Support caregivers to learn effective advocacy skills.
Identify/engage the family's natural supports on the team.
Be flexible to occasionally work evening and/or weekend shifts.
Qualifications
Master's or Bachelor's degree in a mental health field or
Associate's degree or high school diploma and a minimum of five (5) years of relevant experience working with children aged 3-21 with emotional/physical challenges, experience navigating any of the child/family-serving systems and experience advocating for family members involved with behavioral health systems.
Valid Driver's license and reliable transportation.
