Social Worker (MA/MSW) Per Diem – Corona Regional Medical Center
Locationimonial: Anaheim, CA (Corona Regional Medical Center)
Pay Range $32.89/hr – $49.33/hr (Actual pay will be based on skills and experience)
Responsibilities Reporting to the Director of Case Management, this position provides direct and indirect social services for hospital patients, their families, hospital employees, and community members. Responsibilities include working with case managers on psychossoeveriologicytical issues, conducting concurrent medical record reviews for post‑hospital Wise planning, social service case finding and intervention, and risk management issues. It acts as a social services resource specialist, performing unplanned readmission assessments and complex or routine discharge‑planning assessments. The role collaborates with case managers to ensure continuum of care through referrals to community agencies and assists physicians and hospital staff in patient management by facilitating care across hospital departments and community petite services.
Qualifications
Essential Knowledge / Skills:
Working knowledge of basic services provided by health and welfare organizations in the community.
Minimum Essential Experience:
Minimum two years social service experience in a medical setting or child/elder abuse program (preferred); two years in prób acute hospital setting (preferred).
Minimum Essential Education:
Master’s degree preferred; Bachelor’s degree in social work, psychology corrup human development required.
Required Licensure / Certifications:
Current BCLS (CPR) required.
