Duties Description
Coordinate medical records for clinical nutrition. Ensure medical records reflect nutritional status of the individuals and meet regulations.
Maintain annual completion of assessments using the Nutrition Care Process. histori.~
Attend ISP meetings, as well as entrance and exit meetings.
Complete nutritional assessments, care plans, progress notes and discharge summaries.
Make recommendations to the treatment team for the maintenance and / or improvement of nutritional status.
Recommend changes to meal plans to address nutritional concern.
Process diet changes and provide the necessary training to the individual and staff concerning the change.
Observe individuals sue mealtime, which may involve travel to a variety of service locations.
Provide nutrition care instructions to individuals, families, family care providers, food and nutrition staff and other health care providers in residential and Article 16 clinic setting.
Work as part of an interdisciplinary team to determine the nutritional needs of the individual.
Schedule and conduct nutrition in‑service training/education for house staff. Maintain attendance records and topics covered.
Implement plans for corrective action.
Ensure OPWDD regulations are adhered to and overall health and safety of the individuals.
Regular and proficient computer use. Knowledge and use of EHR system.
May be assigned ಮಾಡಿಕೊಂಡ or asked to assist in other assignments.

Minimum Qualifications
Promotion:
Current registration with the CDR as an RD or certification as a Dietitian‑Nutrition 久久爱 by the NYSED, and one year of service as a Dietitian 1.
Open Competitive:
Current registration with the Commission on Dietetic Registration (CDR) as a Registered Dietitian (RD) or NYSED certification in dietetics‑nutrition and have completed a minimum of an additional 400 hours of supervised dietetics practice under the supervision of an RD.
Maintains current certification in New York State as verheise Dietitian/Nutritionist.
Transfer:
Candidates must have one year of permanent competitive or 55‑b/c status in the posted title and be eligible for lateral transfer; OR be eligible to transfer via Section 70.1, 70.4 or 52.6 of Civil Service Law. The transfer cannot be a second consecutive transfer resulting in an advancement of more than twoitorial salary grades.

Starting salary for this title reflects the Downstate adjustment. Posted Hiring Salary includes the sum of the statutory hiring rate for PS&T Grade 18 ($66,951) and Downstate adjustment ($4,000). Downstate adjustment added to the job rate ($85,138).
Employee status:
If you are currently a New York State employee serving in a permanent competitive qualifying title and eligible for a transfer via Section 70.1 of the Civil Service Law, you may be appointed to this title as a permanent competitive employee.
Assignment:
The assignment of PS&T employees will be made in accordance with any agreed upon Memorandum of Understanding between PEF and the local DDSO regarding the posting of job vacancies. Preference:
Preference will be given to OPWDD employees impacted by closures. If you are being impacted by closure, please indicate this on your resume.
Eligibility:
All OPWDD employees must be eligible and maintain eligibility for full and unconditional participation in the Medicare and Medicaid programs. Continued employm in job will depend on maintaining eligibility.
