Position Overview
Community Health Worker (CalAIM Program)
Location:
Monterey, CA
Compensation:
$25–$30/hour
Schedule:
Monday–Friday: 8:00 AM – 3:00 PM
An organization is seeking experienced Community Health Workers to support outreach, enrollment, and ongoing member services for the CalAIM program. This role blends community outreach and enrollment with case management–style responsibilities and is best suited for individuals who are self-directed, comfortable working in the field, and experienced with CalAIM or similar programs.
Key Responsibilities
Conduct community outreach to educate individuals on CalAIM services and enroll eligible members
Coordinate and staff outreach tables at clinics, hospitals, and community-based events
Build relationships with community partners and facilities to support outreach efforts
Manage a caseload of up to 50 members (average caseload 25–30)
Conduct member assessments and manage incoming referrals
Provide case management support, including housing navigation as needed
Travel within the community to meet members and conduct outreach
Required Qualifications
Prior outreach and/or enrollment experience with the CalAIM program or similar community-based programs
Ability to work independently in the field with minimal supervision
Reliable transportation and willingness to travel locally
Bachelor’s degree (preferably in Social Work, Psychology, Sociology, or related field) with 2+ years of relevant experience OR no bachelor’s degree with 4–5 years of direct community health, outreach, or enrollment experience
Preferred Qualifications
Community Health Worker (CHW) Certificate
Existing community outreach network or experience coordinating outreach locations
Self-motivated and proactive with a strong outreach mindset
Comfortable engaging directly with community members
Organized and able to balance outreach goals with case management responsibilities
Experience working in community-based settings
