We are seeking an enthusiastic, efficient, and self‑motivated Registered Nurse to join our outstanding medical team. The ideal candidate is a dependable team player with a positive attitude and a strong commitment to providing exceptional care to our renal patients. This role requires excellent communication skills and the ability to interact professionally with both patients and providers. Accuracy and attention to detail are essential to ensure the highest standards of patient care.
Responsibilities
Phone Triage: Assess and manage patient concerns over the phone, asking pertinent questions to evaluate the urgency and severity of symptoms. Evaluate and educate, schedule appointments, or direct patients to emergency services as needed.
Coordinate established patient hospital follow‑up patients, including scheduling appointments, ordering necessary lab tests, and collecting demographic, insurance and medical records.
Maintain thorough and complete records in patient electronic health records (EHR), ensuring seamless continuity of care and accurate medical records.
Process medication refills, including obtaining prior authorizations when necessary.
Evaluate lab results, contact patients to discuss medications and diet, provide education about their condition, and schedule follow‑up labs as needed.
Receive orders from providers and implement care plans, including infusion orders, initiating and/or titrating medications, educating patients on their conditions, triaging symptoms, and monitoring lab trends.
Coordinate referrals for further testing, imaging, or biopsies with the appropriate facilities, ensuring prior authorizations are obtained when necessary.
Perform Natera swaps for genetic testing.
Adhere to all relevant regulations and standards, including HIPAA and state licensing requirements.
Perform additional tasks or responsibilities as requested by management to support business needs and team objectives.
Qualifications
Valid Registered Nurse (RN) license and current certification in BLS/CPR.
Minimum of two years of relevant inpatient nursing experience preferred.
Previous experience in nephrology or a dialysis center is highly desirable.
Previous experience using EPIC is preferred.
Skills and Abilities
Excellent verbal and written communication skills to interact effectively with patients, team members, and physicians.
Ability to provide exceptional patient care with empathy and professionalism.
Knowledge of nephrology diagnoses, appropriate lab and imaging orders, various treatment options, and the varying levels or urgency for patient care.
Precision and meticulous attention to detail to ensure the accuracy of medical records.
Ability to multi‑task and prioritize effectively to deliver high‑quality, patient‑centered care.
Self‑motivated and able to stay productive without constant supervision.
