Pay: $17–$21 per hour, depending on experience
About The Role Sterling Health Solutions is seeking a highly organized, personable, and dependable Patient Care Coordination & Scheduling Specialist to join our growing Transitional Care Management team. This role works closely alongside our Chief Operating Officer (COO) and plays a critical part in ensuring smooth patient care coordination, efficient provider scheduling, and exceptional communication across patients, clinicians, and internal leadership. Key Responsibilities
Coordinate patient care activities to ensure timely and appropriate services
Schedule patient visits with clinicians and healthcare providers
Manage and maintain provider calendars, adjusting schedules as needed
Serve as a primary point of contact for patients, ensuring a positive and supportive experience
Communicate clearly and professionally with patients, providers, and internal teams
Support the COO with operational coordination related to patient flow and scheduling
Maintain accurate scheduling records and documentation
Respond promptly and professionally to patient and provider inquiries
Required Qualifications
Strong customer service skills with a patient-first mindset
Excellent verbal and written communication skills
Highly organized with strong attention to detail
Ability to multitask and prioritize in a fast-paced environment
Comfortable working directly with executive leadership
Professional, empathetic, reliable, and team-oriented
Preferred Qualifications
Experience with Transitional Care Management, Home Health, or healthcare operations
Experience in healthcare, patient coordination, or scheduling
Familiarity with provider scheduling systems or electronic health records (EHRs)
