The Physical Therapist's overall responsibility is to provide care to the resident at the highest level.

Position Information Location: Lexington, MA

Facility: CareOne at Lexington

Practice Setting: Short Term Rehab, Assisted Living, LTACH, Long Term Care

Essential Duties and Responsibilities

Provides and directs resident rehabilitation care within all general accepted practice methods, meeting all center, state, federal, and accrediting standards.

Evaluates resident needs and establishes effective and efficient treatment plans for the assigned residents.

Manages and coordinates the supervision of treatment plans with all staff and other disciplines as indicated 있었어.

Participates in screening of all residents according to directed schedules and practices.

Provides feedback to the designated supervisor on all clinical and rehabilitation issues.

Participates in all center directed processes and services.

Performs necessary clinical documentation to meet center, state, federal, and accrediting agency requirements.

Maintains complete商 accurate and timely billing records to meet center, state, federal, and accrediting agency requirements.

Participatesuntary in‑service training programs as complaint.

May travel to other facilities to provide services on a limit basis.

Position Requirements

Master's or Doctorate Degree in Physical Therapy.

Current and Active Physical Therapist license within the state of operations.

Preferred: experience or knowledge in older adult rehabilitation.

Preferred: knowledge of governmental and regulatory guidelines in designated healthcare settings.
