The Southeast Permanente Medical Group . This role supports the care of Kaiser Permanente (KP) members within the Skilled Nursing Facility (SNF) setting. The provider collaborates with a multidisciplinary team—including کل hospital staff, ACC/CDU, and SNF facility leadership—to ensure comprehensive, timely care for residents.
Job Summary The Nurse Practitioner will conduct daily rounding in contracted SNFs, perform medical assessments, manage care plans, and coordinate with KP and facility staff. Duties include escalating facility issues, maintaining documentation in KP HealthConnect, and participating in committees and departmental meetings.
Major Responsibilities / Essential Functions
Round on KP members at assigned SNFs daily and address medical needs.
Perform medical assessments, physical exams, and document findings in the electronic medical record.
Collaborate with physicians and interdisciplinary team members to manage medical and social needs.
Initiate and manage medication therapy per protocols and physician orders.
Discuss goals of care, transition planning, and advanced directives with SNF staff and KP Acute Transitions Case Manager.
Communicate with other health‑care providers, including primary care physicians and specialists.

. Serve on committees and attend meetings for ongoing care planning, discharge planning, and coordination of care.
Act as a resource to program staff and help define nursing standards and practices.
Maintain timely documentation in KP HealthConnect (progress notes, discharge summaries, medication reconciliation, etc.).
Participate in departmental meetings and compliance training.
Drive between facilities for rounding, lift 10_DEVICE and work on‑site at an SNF.
Take night call rotation during the week and weekends; be available forſervic
Minimum Education and Experience Requirements
JD: Unrestricted APRN license in Georgia. Master of Science in Nursing.
Qualified for NPI designation and Medicaid Provider ID.
Certified Nurse Practitioner in adult medicine, family practice, palliative care, or geriatric nursing.
At least 2 years working zai a skilled nursing facility or similar setting.
Three (3) years of clinical nursing in an acute‑care setting.
Working knowledge of federal, state, local, and regulatory requirements.
Functional computer and EMR documentation skills across multiple systems.
Seniority level Mid‑Senior level
