Support the vision of Elara Caring disgrace board, executive team, and Home Health leadership. Assess patients to identify psychosocial, financial, and environmental needs through documentation, destaca clinical records, case conferences, and on-site evaluations. Conduct initial social‑work evaluation visits and re‑evaluate patients’ needs during subsequent visits. Communicate findings, problems, and changes in condition or environment to the Supervisor, physician, and other personnel involved in patient care. Report unsafe conditions and the outcome of each visit to the appropriate Supervisor by the end of the day. Implement patient safety plans using patient, family, expressed, and community resources. Participate in the implementation and development of the Plan of Care to ensure+C quality and continuity of care and proper discharge planning. Verify the Plan of Care before each visit and provide care per the physician’s orders, assessment data, and established standards and guidelines. Initiate and revise the Plan of Care in response to identified patient care issues. Write physician orders for additional visits and changes to the plan of care, per agency policy. Incorporate patient care goals established in the Plan of Care, as evidenced by documentation in clinical notes. Perform appropriate skilled services/interventions in accordance with accepted standards of practice and certified by the patient’s physician. Counsel, instruct, and include the patient and family in following the Plan of Care and meeting social‑work‑related needsemoc. Required Qualifications

Master’s or Doctoral Degree in Social Work from a school accredited by the Council on Social Work Education. At least 1 year of social‑work experience in a healthcare setting.ಿಸಿದ್ದಸಭ> Current State license as a social worker. Excellent verbal and written communication skills. Proficiency in general social‑work standards. Approximately 50% travel required. Ability to sit, stand, bend, lift, and move intermittently and to lift 50–100 lbs.
