to join our
Home Health & Hospice Department
in
Galion, Ohio .
Position Overview The Home Health and Hospice Registered Nurse provides nursing care to patients as prescribed by the Physician and requested by the patient/caregiver within the framework of the plan of treatment/care in compliance with applicable laws and regulations and the policies of the organization.
Responsibilities
Visit patients in their homes for one-on-one visits
Complete initial and ongoing assessments to identify the physical, psychosocial, and environmental needs of home health patients
Assess patients’ conditions, chart observations, and perform evaluation tasks including vital signs and medication review
Administer medication as prescribed by the patient’s physician
Educate patients and families on proper home health care strategies and procedures, and recommend devices or tools that may improve quality of life
Provide effective communication to patients/clients, families, team members, and other health care professionals
Qualifications Required:
Associate’s Degree (A.D.), Diploma, Bachelor’s Degree (B.S.), or Master of Science (M.S.) from an approved nursing school
Current State of Ohio Registered Nurse licensure
BLS certification or obtained within 30 days of hire
Preferred:
One year of nursing experience
Previous experience in community, health, home health, hospice, or appropriate specialty area
