Brief Job Description Provider Investigations Inspector – Starting Market Rate Salary $ 60,431.88.

In this front‑line field position, the Inspector V protects residents’ or individuals’ health and safety by conducting on‑site inspections and investigations – including abuse, neglect, and exploitation of adults and children with mental illness or intellectual, developmental, and physical disabilities in various Texas Long‑Term Care facilities and providers (Assisted Living Facilities, Day Activity and Health Services sites, DAHS‑ISS sites, Prescribed Pediatric Extended Care Centers, State Hospitals, Managed Care, and Community IDD services). Investigations occur in facilities and private residences, and are completed in accordance with Texas Administrative Code and Long‑Term Care Regulatory policy.

The position may require testimony at hearings or court proceedings to provide information regarding policy, procedures, history , findings, or activities. The inspector is mobile and flexible, performs minimal work at an office, and must write reports.

Responsibilities

Conduct interviews concerning allegations of abuse, neglect, or exploitation.

Collect evidence pertinent to the investigation.

Use critical thinking and sound judgment to analyze evidence related to the allegation.

Write reports summarizing the facts obtained رات during investigations, indicating whether bula occurred.

Use personal or rental vehicles for travel in accordance with state travel policy; travel reimbursement provided.

Registrations, Licensure Requirements or Certifications

Valid driver’s license.

Initial Screening Criteria

Experience interacting with or communicating with members of the public, in person or by phone.

At minimum, the applicant must have 60 hours of college credits from an accredited college or university.

Four‑year degree from an accredited college or university, preferred.

Experience conducting investigations preferred.

Experience working with individuals with mental illness, intellectual or developmental disabilities, or physical disabilities preferred.

Additional Information Flexibility in work hours is required.Gone the job requires 80% travel, including statewide travel, overnight and weekend travel. On‑call duties for emergencies or high‑priority situations may be required. Must have reliable transportation صاحب.
