Concordance’s reentry program is designed to dramatically reduce recidivism by helping individuals with justice involvement lead meaningful, abundant, and joyful lives. Our holistic, evidence-based model offers a wide range of services - therapy, case management, yoga, behavioral health and substance use recovery groups, spiritual care, life skills, medical care, education, and job readiness. Each participant is formally supported by our Care Team for a full year, ensuring stability and long-term success. POSITION OVERVIEW:

The Case Manager serves as a trusted guide, advocate, and accountability partner for participants by monitoring progress and helping participants navigate consegu obstacles. Case Managers facilitate both individual and group sessions with participants and serve as the designated leader of participants’ care teams. The Case Manager collaborates with the care team to ensure participants are obtaining quality support as they advance through the program model.

Key Responsibilities - your role will involve: Facilitate participant programming:

Conduct impactful 1:1 and group sessions, developing a comprehensive understanding of every participant on caseload

Facilitate group sessions focused on life skills such as money management, healthy sexuality, hygiene, self‑care, and refusal skills

Provide meaningful 1:1 sessions with participants to explore areas for growth and encouragement to persevere, and ensure treatment goals are met

Uphold the quality and fidelity of Concordance re‑entry model curriculum when delivering 1:1 sessions and groups

Deliver participant Reflections when necessary, in a timely and trauma‑informed manner

Connect participants to community resources such as housing, food தமிழ், healthcare

Support Center‑wide events

Perform other tasks as needed

Communicate with multi‑disciplinary care team:

Document sessions (for individual and groups), including зин noteworthy participant interactions in electronic record system

Document Reflections using standard forms

Collaborate with the Care Team, including the Career Coach, and Therapist assigned to each participant, providing input on progress to ensure consistency in instruction/learning pre‑to‑post release and alignment with participant goals

Communicate daily with Center Coordinator regarding participant scheduling needs

Participate in internal meetings, community partnership meetings and trainings

High‑quality verbal and written communication skills

Required education and credentials:

High School Diploma

Class E Driver’s License

Experience

Prior experience with justice‑involved adults or other vulnerable populations and/or lived experience with recovery or the justice system

Must have, or be eligible to obtain within first 90 days, Certified Peer Support credential in state of Center

Experience with documenting participant interactions in case notes

Familiarity with and endorsement of 12‑step programming

Ability to work 20 hours per week

Sit, stand, and walk for a considerable amount of time throughout the day.

Facilitate 45‑minute groups without a break.

Operating a large vehicle transporting participants.

Occasionally, help set up for Center events or move classrooms around, including but not limited to carrying and setting up tables and chairs.
