has an opening for a Primary Care Nurse Practitioner to provide quality and flexible medication management and behavioral health services to child, adolescent, and adult clients with serious mental illness (SMI), severe emotional disturbance (SED), and substance use diagnoses. This position will be primarily based at NorthKey's outpatient office located in Covington, KY with a rotation to our Carrollton office and
Mobile Unit. This is an on-site position, and the hours are M-F 8:30 to 5 pm (Possibility for FOUR 10's). NO EVENINGS, WEEKENDS, and 10 PAID holidays!! This position will allow that perfect work and life balance!

QUALIFICATIONS
Master's degree or above in a nursing field of study from an accredited school.
Relevant experience, knowledge, and expertise in the behavioral health field.
Must have experience working in Primary Care
An active DEA is highly desirable.
JOB DUTIES
Provides medication management services based upon initial and ongoing assessments of child, adolescent, and adult clients with SMI, SED, and SUD diagnoses.
Develops individual service plans based upon assessment data, and provides appropriate medication interventions as necessary to resolve presenting symptoms.
Performs patient history screenings, risk factor assessments, and ordering of therapeutic and diagnostic tests as indicated by client status.
Appropriately prescribes medications following organizational policies and relevant federal/state regulations.
Consults with Lead Medical Officer on cases that are beyond the scope of knowledge and expertise.
Adheres to appropriate standards of care, and ensures clients are seen routinely prior to providing medication refills.
Responsible for monitoring individuals on caseload for changes or decline in mental status or medical conditions.
Work independently with other members of the professional staff to promote quality and effective patient care.
Completes quality documentation of services provided, including service plans, progress notes, medication informed consent, treatment plan reviews, and other required information to assure compliance with organizational policies and third-party compliance standards.
Participate in staffing meetings and consultation with other clinicians to assess therapeutic needs of clients and encourage appropriate referrals to internal and external community services.
Maintain communication and positive multi-disciplinary working relationships with clinicians and other related professionals in the community. This will include making appropriate referrals, sharing clinical information within HIPAA guidelines, and providing clinically appropriate interventions and collaborations.
Complete timely and accurate documentation within an electronic health record, prepare and complete progress notes, assessments, reports, schedules, and other documentation as required of the position.
Attend and participate in relevant continuing education and in-service training to improve professional competency and clinical intervention skills.
Maintain flexible hours to meet agency and client needs.
This position may perform services in the outpatient office, school, or other external setting as needed.
