Job Overview We are seeking an experienced Case Manager for our new men’s shelter, Liberty Plaza. Case Managers are part of an interdisciplinary team aimed at stabilizing shelter clients by improving their independent living skills and assisting clients in moving toward their housing goals. They assess client needs, link clients to services, plan a course of action toward stabilization and re‑housing, monitor progress, and document client contacts. The successful candidate will have prior experience working with homeless and/or mental health individuals.

Essential Functions

Assess treatment needs and develop, monitor, and evaluate treatment plans.

Determine clients’ requirements by completing intake interviews and ascertaining mental and medical needs.

Establish treatment programs by setting schedules and routines, coordinating services, and arranging resources, including transportation and escort.

Monitor cases by verifying attendance, observing and evaluating treatments and responses, advocating for needed services and entitlements, obtaining additional resources, intervening in crises, and providing personal support.

Communicate clients’ progress through weekly interdisciplinary meetings and evaluations; disseminate results and obstacles to the therapeutic team and family; identify treatment influences.

Improve treatment results by studying, evaluating, and re‑designing processes; implementing changes; rewriting policies and procedures.

Requirements

High school diploma, HSE, or GED required.

Minimum of 2 years of social services experience required.

Possession of a valid Food Handler’s License, or ability to obtain one within six (6) months of hire, is required.

Experience working with the homeless, mentally ill, substance abuse, and/or low‑income populations.

Strong relationship‑building and computer skills required, including Word and Excel.

Ability to work evenings and/or weekends on a rotating schedule.

Preferred experience working in a homeless shelter.

Knowledge of mental health, substance use, or homelessness.

Bilingual (English/Spanish) preferred.

Work Environment This job operates in a shelter environment and works with a population who may suffer from mental illness, substance abuse, disability, HIV/AIDS, and/or other health issues.

Physical Demands Regularly required to stand, walk, sit, stoop, climb stairs, and/or kneel. Occasionally required to lift and/or move up to 10 pounds.
