Patients trust us to receive the right personalized care where and when they need it – in our care centers, at home or virtually – to improve their health outcomes and quality of life.* * Provide comprehensive primary and acute care to patients in Assisted Living, Memory Care, Hospice, and Group Home settings in Maricopa County.* Order, interpret, and evaluate diagnostic tests to assess and manage patients’ health conditions.* Deliver health education, counseling, and disease prevention guidance to promote wellness.* Collaborate with physicians, nurse practitioners, and other healthcare professionals as part of an interdisciplinary care team.* Develop, document, and update individualized care plans.* Prescribe medications and other appropriate treatments in accordance with licensure and scope of practice.**Requirements:*** Requires an MS in Nursing* Minimum of 1 year of related experience with clinical emphasis for nurse practitioner; or any combination of education and experience, which would provide an equivalent background.* Current unrestricted RN license and NP license in applicable state required.* Satisfactory completion of a Tuberculosis test is a requirement for this position.*\*\*This position is bonus eligible based on individual and company performance.\*\****Compensation:**$108,466.00to$135,582.00At CareMore Health, we provide effective Advanced Primary Care and Palliative Care by seeing the whole person — body, mind, and spirit.
