Summary All About Home Care is a Houston‑based home health care agency that has been serving the Houston and surrounding communities since 2003. We are currently looking for a Registered Nurse (RN) to help serve patients in the North East of Greater Houston area. If you have experience in home health and are interested in covering this area, please submit your resume. We use Kinnser software to document patient care.
Qualifications
Graduate of an approved school of professional nursing and currently licensed in the state of Texas.
Home Health skilled nursing experience (RN).
High clinical skills with emphasis on wound care, VAC, and IV.
RN must have OASIS experience.
Ability to exercise initiative and independent judgment.
Responsibilities
Provide services in accordance with the plan of care.
Make the initial evaluation visit and regularly reevaluate the patient’s nursing needs.
Provide services requiring substantial specialized nursing skills.
Initiate appropriate preventative and rehabilitative nursing skills.
Prepare clinical and progress notes for each patient’s visit and summaries of care conferences in a timely manner as per agency policy.
Inform personnel of changes in the conditions and needs of the patient.
Counsel the patient and the family/significant others in meeting nursing and related needs.
Participate in and present in‑service programs.
Understand and adhere to established agency policies and procedures.
Process orders and notify physician of patient needs and changes in condition.
Complete certification/re‑certification orders and discharge summaries.
Determine the amount and type of nursing needed by each individual patient.
Refer patients requiring specialized skills to Physical Therapist, Speech Language Pathologist, Occupational Therapist, and Medical Social Worker.
Supervise and teach other nursing personnel.
Conduct patient care conferences on patients assigned to your care.
Participate in peer review and performance improvement as assigned.
Provide total patient care as needed.
