Your Role – What You’ll Do
Be
fully responsible for on-call coverage
during your 7 days on (nights, days, weekends) Respond to urgent patient needs including pain, symptom management, and crisis situations Provide telephonic support to families and caregivers with empathy and clarity Conduct in-home visits as needed across our 21-county service area Coordinate with the IDG to ensure appropriate follow-up and care planning Maintain accurate, timely documentation in our electronic health record
Qualifications – What You Bring
Current MN Registered Nurse license Previous hospice, oncology, or palliative care experience required Clinical confidence to triage and respond to a wide range of scenarios Excellent communication and critical thinking skills Ability to self-manage a demanding on-call schedule with professionalism Reliable vehicle, valid driver’s license, and comfort driving to homes throughout the metro Personal cell phone for work communication ( device not provided; reimbursement available )
