You’ll provide both home health and infusion services with a focus on high-quality, patient-centered care. Key Responsibilities Administer infusion therapies and medications Perform trach care, enteral feedings, wound care, and med management Monitor health status and respond to changes Maintain accurate and compliant documentation Deliver care based on individualized Plans of Care Educate patients and families to promote confidence and independence

*Travel may be required within a 30-mile radius. This position offers meaningful, hands-on experience in home-based care.* What We're Looking For Current, unrestricted California RN license Proficiency with IVs, ports, PICC lines, and home health procedures Strong clinical skills including trach care, feeding tubes, wound care, and medication routines Thorough understanding of HIPAA and Joint Commission standards Reliable transportation and valid auto insurance Clear background check Advanced certifications (CRNI, OCN) are a plus.
