What to Expect (Job Responsibilities): - Lead assigned patient service lines and therapeutic programs - Monitor patient care, provide education, and manage medication reconciliation - Coordinate specialty medication access and affordability prior to therapy initiation - Deliver direct patient care and participate in pharmacy operations - Facilitate experiential education and training for pharmacy students and residents
What is Required (Qualifications): - Graduate of an ACPE accredited School of Pharmacy - Completion of ASHP accredited PGY-1 Pharmacy Residency - Strong written and verbal communication skills - Significant clinical knowledge and expertise in patient-centered care - Eligible for licensure as a pharmacist in the state of Illinois
How to Stand Out (Preferred Qualifications): - Doctor of Pharmacy or another advanced degree - Completion of PGY-2 Pharmacy Residency - 2-4 years of clinical pharmacy experience in a hospital setting - Pediatric experience
