The Wraparound Facilitator’s primary responsibility is to facilitate Child and Family Planning Meetings using Wraparound principles that are family centered, individualized, strengths based, and outcome focused. The program’s goal is to help children and families with complex and enduring needs receive intensive treatment in the least restrictive environment that is appropriate. The Wraparound Facilitator’s role is to provide assessment of the client and family’s needs and lead the team in providing treatment to support the family toward meeting their goals. RESPONSIBILITIES Facilitate Child and Family Planning Meetings, including preparation, coordination, documentation, and follow-up Provide case management and care coordination in collaboration with families and multidisciplinary partners (e.g., county workers, therapists, educators) Maintain timely and accurate documentation, including mental health notes, assessments, treatment plans, incident reports, and meeting summaries Support clients through behavior intervention and crisis response, including de-escalation and physical management when necessary Participate in team meetings, trainings, and clinical supervision, and adhere to best clinical practices Maintain a flexible schedule, including early mornings, evenings, weekends, and on-call shifts as needed Model professional conduct and uphold the values and mission of the program and agency Take on leadership responsibilities and contribute to a positive, supportive team culture QUALIFICATIONS REQUIRED: Bachelor’s degree in Counseling/Psychology/Social Work or related field 2 years 'of experience working with children/youth and families with complex and enduring needs in a mental health setting Flexible schedule with the ability to work some evenings and weekend hours as needed Be part of a rotating emergency on-call system, scheduled ahead of time At least 21 years of age TB test clearance, fingerprinting clearance, and any other state/federal licensing or certification requirements Valid driver's license, clean driving record, and insurability through Seneca Family of Agencies’ insurance policy PREFFERED: MHRS Certification Bilingual Spanish skill set Excellent organizational and critical-thinking skills SCHEDULE Full-time; Monday - Friday, 9am - 5:30pm BENEFITS Starting at $39.85 - $42.35 per year, commensurate with experience Additional compensation provided upon passing bilingual language proficiency exam Salary increases each year
