Description Assess patients in either inpatient or outpatient settings and create personalized treatment plans tailored to their needs Implement physical therapy interventions emphasizing mobility, strength, balance, and overall functional recovery Collaborate closely with nurses, physicians, case managers, and other rehabilitation specialists to enhance patient outcomes Guide patients and their families through therapeutic objectives, discharge protocols, and necessary home exercises Meticulously document evaluations, treatments, and patient progress in accordance with all facility and regulatory standards Engage in interdisciplinary team meetings and rounds to ensure cohesive and coordinated patient care Education
Doctorate in Physical Therapy (DPT) or an equivalent qualification from a recognized program Certifications
Valid state licensure as a
Physical Therapist CPR certification is preferred Additional specialized certifications (e.g., neurological, geriatric areas) are valued but not mandatory Skills
Relevant experience is highly favored Proficient in treating patients with complex medical and mobility concerns Excellent communication skills and a collaborative spirit Familiar with digital charting and clinical documentation tools Benefits
