The Opportunity: As a Field Nurse (RN) with Executive Home Care, you'll be vital to our patients' well-being, providing comprehensive nursing care and support in the comfort of their homes. This role offers professional independence, direct patient impact, and the satisfaction of building meaningful relationships.
Key Responsibilities:
Comprehensive Patient Assessment: Conduct thorough initial and ongoing assessments of patients' physical, mental, and emotional health, identify needs, and develop individualized care plans.
Medication Management: Assist with medication administration and ensure proper adherence to prescribed regimens.
Personal Care Assistance: Oversee and provide guidance on activities of daily living (ADLs), such as bathing, dressing, and feeding, to ensure patient comfort and dignity.
Vital Signs Monitoring: Regularly monitor and assess vital signs, reporting any significant changes to the appropriate healthcare professionals.
Health Education & Support: Educate patients and their families on general health and wellness, disease prevention, and self-care techniques to promote independence and optimal health outcomes.
Documentation & Communication: Maintain accurate, timely, and detailed clinical documentation in electronic health records (EHRs). Communicate effectively with the interdisciplinary team, physicians, and families to ensure coordinated and holistic care.
Care Coordination: Act as a liaison between patients, physicians, and other healthcare providers to ensure seamless transitions of care and access to necessary resources.
Emergency Response: Respond to changes in patient condition and medical emergencies, initiating appropriate interventions and seeking medical assistance as needed.
Adherence to Standards: Adhere to all professional nursing standards, ethical guidelines, and company policies and procedures, including HIPAA regulations.
Professional Development: Participate in ongoing professional development and continuing education to maintain licensure and enhance clinical skills.
Qualifications:
Valid and unrestricted Registered Nurse (RN) license in New Jersey.
Associate's Degree in Nursing (ADN) required; Bachelor of Science in Nursing (BSN) preferred.
Minimum of 3 years of recent clinical experience as an RN, with at least 3 years in home care, long-term care, or a similar community-based setting.
Current CPR certification (BLS required).
Valid driver's license, reliable transportation, and auto insurance.
Excellent clinical assessment, critical thinking, and problem-solving skills.
Strong communication, interpersonal, and organizational skills.
Ability to work independently and manage a caseload effectively.
Proficiency in using electronic health records (EHRs) and other relevant technology.

Ability to adapt to various home environments and patient situations.
