The individual must show knowledge of the principles of growth and development across the lifespan. They must possess the ability to assess data reflective of the patient's status and interpret appropriate information needed to identify each patient's requirements. The LPN is responsible for providing direct and indirect skilled nursing care for assigned residents. They will follow defined policies and procedures under the supervision of a Registered Nurse.The LPN will maintain a collaborative relationship with other nursing staff, physicians, and ancillary staff. It is vital for the LPN to maintain an appropriate level of knowledge and clinical expertise through continuing education and in-service programs. The LPN shall have the ability to provide care to geriatric residents.Job Duties:Collect data relevant to the resident's physical and psychosocial condition on an ongoing basis. Communicate findings to the Registered Nurse and other members of the Interdisciplinary Team. Contribute to the creation of a developmentally age-appropriate and culturally relevant plan of resident care. Reflect nursing diagnoses and interventions to achieve desired resident outcomes. Involve the resident/family and healthcare team in this process while promoting a healthy, safe environment. Recommend revisions to the plan of care after evaluating resident outcomes. Indicate the need for referrals to ensure all aspects of the resident's care have been addressed. Collaborate with the resident/family and members of the Interdisciplinary Team within the scope of nursing practice. Ensure customer satisfaction. Maintain current knowledge and competency in Nursing Practice. Participate in the mentoring/orientation of staff. Consider factors related to safety, cost, and impact in planning the delivery of care. Under the direction of the RN, provide leadership in the professional practice setting. Demonstrate knowledge and competency. Understand that occupational exposure to bloodborne pathogens is reasonably anticipated. Required tasks involve potential contact with blood, body fluids, or tissues.
