The individual must demonstrate knowledge of the principles of growth and development across the lifespan. The LPN should possess the ability to assess data reflective of the patient's status and interpret the appropriate information needed to identify each patient's requirements relative to their age-specific needs. The role includes providing direct and indirect skilled nursing care for assigned residents. The LPN follows defined policies and procedures under the supervision of a registered nurse. The LPN maintains a collaborative relationship with other nursing staff, physicians, and ancillary staff. The LPN is responsible for maintaining an appropriate level of knowledge and clinical expertise through continuing education and in-service programs. The ability to provide care to geriatric residents is essential. Job Duties: Collect data relevant to the resident's physical and psychosocial condition on an ongoing basis and communicate findings to the registered nurse and other members of the interdisciplinary team. Contribute to the creation of a developmentally age-appropriate and culturally relevant plan of resident care. Reflect nursing diagnosis and interventions to achieve desired resident outcomes while involving the resident and family in this process. Promote a healthy, safe environment. Recommend revisions to the plan of care after evaluating resident outcomes and indicate the need for referrals to address all aspects of the resident's care. Collaborate with the resident, family, and members of the interdisciplinary team within the scope of nursing practice. Assure customer satisfaction. Maintain current knowledge and competency in nursing practice. Participate in the mentoring and orientation of staff. Consider safety, cost, and impact in planning the delivery of care. Under the direction of the registered nurse, provide leadership in the professional practice setting. Demonstrate knowledge and competency in nursing practices. Occupational exposure to bloodborne pathogens is reasonably anticipated. Required tasks involve potential contact with blood, body fluids, or tissues.
